XP 95 Disconnected fault

Got a site with a few xp95 heads now and again panel will show disconnected faults for a random device - usually i find warped bases to be the problem on but can only clear the fault with a new device.

Does anyone know if these detectors usually (ion / opt) die for no apparent reason?
